CHILDREN ACT 2004

This item is being used for development purposes and has not yet been assured by the Information Standards Board for Health and Social Care.

The Children Act 2004 provides a legislative spine for the wider strategy for improving children's lives. This covers the universal services which every child accesses, and more targeted services for those with additional needs.

The overall aim is to encourage integrated planning, commissioning and delivery of services as well as improve multi-disciplinary working, remove duplication, increase accountability and improve the coordination of individual and joint inspections in Local Authorities. The legislation is enabling rather than prescriptive and provides Local Authorities with a considerable amount of flexibility in the way they implement its provisions.

The Children Act 2004 places a duty on Local Authorities to promote the educational achievement of Looked After Children.

This item is being used for development purposes and has not yet been assured by the Information Standards Board for Health and Social Care.

The data carried in the three National Service Framework for Children Young People and Maternity Services Data Sets (Maternity Data Set, National Children's and Young People's Health Services Data Set and Child and Adolescent Mental Health Services Data Set) must be structured in a particular way for submission nationally. Data items in the files, including the items in the header records, are pipe-delimited (| character).

Each data set submission file of data is structured in the following way:

PRIMARY CARE TRUST

Primary Care Trust is an ORGANISATION.

A Primary Care Trust is a legal entity, set up by order of the Secretary of State. It is a free-standing NHS body, performance managed by a Strategic Health Authority.

The overall function of a Primary Care Trust is to improve the health of the responsible population, develop primary and community health services, and commission secondary care services. A Primary Care Trust will, if it so wishes and is capable of doing so, be able to provide directly a range of community health services, creating new opportunities to integrate primary and community health services as well as health and social care provision.

The Primary Care Trust's responsible population comprises:

- all PERSONS registered with a General Medical Practitioner Practice whose practice forms part of the Primary Care Trust, regardless of where the PERSON is resident, plus
- any PERSONS not registered with a General Medical Practitioner Practice who are resident within the Primary Care Trust's statutory geographical boundary

Note that PERSONS resident within the Primary Care Trust GEOGRAPHIC AREA, but registered with a General Medical Practitioner Practice belonging to another Primary Care Trust, are the responsibility of that other Primary Care Trust.

With "Shifting the Balance of Power", Primary Care Trusts will be the leading NHS ORGANISATION for partnership with local authorities and a range of other partners, including NHS Trusts, Strategic Health Authorities and a range of other Primary Care Trusts and local communities to improve health and deliver wider objectives for social and economic regeneration.

Primary Care Trusts provide some services themselves and others through agreement with other ORGANISATIONS. Several Primary Care Trusts may decide to work together to provide certain services. In this case a lead Primary Care Trust will be identified for the group.

There may be occasions when relationships are formed on a larger scale. For example the provision of a highly specialised service, such as specialist cancer or spinal injury services, may be done collaboratively across a population larger even than Strategic Health Authority.

Note:
PERSON GENDER CODE "0 Not Known" is only applicable to "Person Gender Current".

National Codes:

A PRIVATE CONTROLLED DRUG PRESCRIBER CODE is allocated by the NHS Prescription Services to private prescribers who prescribe schedule 2 and 3 controlled drugs that are intended to be dispensed by registered pharmacies.

Note: A registered pharmacy means premises for the time being entered in the register required to be kept under the Medicines Act 1968 by the Registrar of the Royal Pharmaceutical Society of Great Britain.

The GENERAL MEDICAL PRACTICE CODE (PATIENT REGISTRATION) is an ORGANISATION CODE.

This is the code of the GP Practice that the PATIENT is registered with.

Use of default Codes

V81997 should be used when a PATIENT presents who is not currently registered at a GP Practice, but is eligible to be registered should they wish to.

V81998 should be used where a PATIENT should not have a registered practice anyway, due for instance to them having only recently entered the country.

V81999 should be used where it is not possible to determine a PATIENTS registered practice code but it is known that they should have one, or where it is impossible even to determine whether they should or shouldn't have a registered practice (for instance the PATIENT cannot communicate and is unidentified).

The code of the practice is an ORGANISATION CODE.

This is the code of the GENERAL MEDICAL PRACTITIONER specified by the PATIENT.

This is a GENERAL MEDICAL PRACTITIONER within the General Medical Practitioner Practice that the PATIENT is registered.

The General Medical Council allocates all doctors a GENERAL MEDICAL COUNCIL REFERENCE NUMBER on their first contact with the General Medical Council.
Note - when a doctor is registered to practise medicine in the United Kingdom, their details will appear on the "General Medical Council List of Registered Medical Practitioners" (LRMP).

 if the GENERAL PRACTITIONER is working as an assistant, use the code of the responsible GENERAL MEDICAL PRACTITIONER (SPECIFIED);
 if the GENERAL PRACTITIONER is working as a GENERAL MEDICAL PRACTITIONER (SPECIFIED), use the GENERAL PRACTITIONER's GENERAL MEDICAL COUNCIL REFERENCE NUMBER.

A doctor receives a GENERAL MEDICAL COUNCIL REFERENCE NUMBER on qualification. The GENERAL MEDICAL PRACTITIONER code linked to his/her main practice is included on the National Administrative Codes Service (NACS) CD-ROM and the NACS NHSnet website.

The GENERAL MEDICAL PRACTITIONER code is an eight character alphanumeric code, see PERSON IDENTIFIER and GENERAL MEDICAL PRACTITIONER PPD CODE.

When a locum refers, use the code of the GENERAL PRACTITIONER for whom the locum is acting. See GENERAL MEDICAL PRACTITIONER PPD CODE.

For GENERAL PRACTITIONERS working in hospitals, the following codes should be used:

-if the GENERAL PRACTITIONER is working as an assistant, use the code of the responsible consultant;
