Cancer Outcomes and Services Data Set Overview

The Mandatory, Required, Optional, Not included or Pilot in the COSDS Message (M/R/O/X/P) column indicates the recommendation for the inclusion of data.

  • M = Mandatory: this data element is mandatory and the technical process (e.g. submission of the data set, production of output etc.) cannot be completed without this data element being present
  • R = Required: NHS business processes cannot be delivered without this data element
  • O = Optional: the inclusion of this data element is optional as required for local purposes
  • X = Not included in the COSDS XML Schema: the National Cancer Registration and Analysis Service obtains the data from another source, or the item is submitted under another Standard and is included here for reference only
  • P = Pilot: this data element is for use in the SNOMED CT pilot project only at present. Please contact COSDenquiries@phe.gov.uk for further details.

Commissioning Data Sets

The Commissioning Data Set is the basic structure used for the submission of commissioning data to the Secondary Uses Service. Every secondary care provider in England has to send a set of standard Commissioning Data Sets to the Secondary Uses Service which contains details of all the care they have provided.

Each Commissioning Data Set name is in Title Case and where the name of a Data Set is Aqua Blue, it indicates that it is a hyperlink and if clicked on will display the Data Set.

There are three frames to each Commissioning Data Set:

  • "Left Hand Frame": shows the Commissioning Data Set menu.

  • "Middle Frame":
    • This displays the Data Set.
    • The Data Set is split into Data Groups which contain Data Elements grouped into logical sections. For example the ‘CDS Data Group: Patient Identity’ contains Data Elements relating to a Patient’s identity such as NHS NUMBER.
    • If you click on any item in the Data Set, the definition will be displayed in the "Right Hand Frame".

  • "Right Hand Frame":
    • This initially shows an "Overview" page, which provides information relating to the type of Commissioning Data Set.

Commissioning Data Sets

The Commissioning Data Set is the basic structure used for the submission of commissioning data to the Secondary Uses Service. Every secondary care provider in England has to send a set of standard Commissioning Data Sets to the Secondary Uses Service which contains details of all the care they have provided.

Each Commissioning Data Set name is in Title Case and where the name of a Data Set is Aqua Blue, it indicates that it is a hyperlink and if clicked on will display the Data Set.

To access a Commissioning Data Set,:

There are three frames to each Commissioning Data Set:

  • "Left Hand Frame": shows the Commissioning Data Set menu.

  • "Middle Frame":
    • This displays the Data Set.
    • The Data Set is split into Data Groups which contain Data Elements grouped into logical sections. For example the ‘CDS Data Group: Patient Identity’ contains Data Elements relating to a Patient’s identity such as NHS NUMBER.
    • If you click on any item in the Data Set, the definition will be displayed in the "Right Hand Frame".

  • "Right Hand Frame":
    • This initially shows an "Overview" page, which provides information relating to the type of Commissioning Data Set.
    • It displays Data Element definitions viewed from the "Middle Frame".

The NHS Data Model and Dictionary provides a reference point for approved Information Standards and Collections (including Extractions) (ISCEs) to support health care activities within the NHS in England. It has been developed for everyone who is actively involved in the collection of data and the management of information in the NHS.

The NHS Data Model and Dictionary is maintained and published by the NHS Data Model and Dictionary Service and all changes are governed by the Standardisation Committee for Care Information. Changes are published as Information Standards and Collections (including Extractions) Notices (ISCE) and Data Dictionary Change Notices (DDCN).

LOWER LAYER SUPER OUTPUT AREA

A Lower Layer Super Output Area (LSOA) is a GEOGRAPHIC AREA.

Lower Layer Super Output Areas are a geographic hierarchy designed to improve the reporting of small area statistics in England and Wales.

Lower Layer Super Output Areas are built from groups of contiguous Output Areas and have been automatically generated to be as consistent in population size as possible, and typically contain from four to six Output Areas. The Minimum population is 1000 and the mean is 1500.

There is a Lower Layer Super Output Area for each POSTCODE in England and Wales. A pseudo code is available for Scotland, Northern Ireland, Channel Islands and the Isle of Man.

The Organisation Data Service publish files created on their behalf by the Office for National Statistics, which link POSTCODES to the Lower Layer Super Output Area.

METROPOLITAN COUNTY

A Metropolitan County is a GEOGRAPHIC AREA.

The six Metropolitan Counties formed the upper tier of a two-tier local government structure in England between 1974 and 1986, when they were abolished and the district councils became unitary administrations. The Metropolitan County areas are still used for statistical purposes however.

The CONSULTANT CODE is derived from either the GENERAL MEDICAL COUNCIL REFERENCE NUMBER for GENERAL MEDICAL PRACTITIONERS, or the GENERAL DENTAL COUNCIL REGISTRATION NUMBER for GENERAL DENTAL PRACTITIONERS (where the dentist doesn't have a GENERAL MEDICAL COUNCIL REFERENCE NUMBER).

For GENERAL MEDICAL PRACTITIONERS working as CONSULTANTS, the GENERAL MEDICAL PRACTITIONER's GENERAL MEDICAL COUNCIL REFERENCE NUMBER should be used, see data item note for GENERAL MEDICAL PRACTITIONER (SPECIFIED).

In the case of overseas doctors, the default code C9999998 should only be used where no GENERAL MEDICAL COUNCIL REFERENCE NUMBER has been assigned.

All Midwife Episodes are identified in the Admitted Patient Care Commissioning Data Set (CDS) and Hospital Episode Statistics by a pseudo MAIN SPECIALTY CODE, 560, see Main Specialty and Treatment Function Codes Table. A default code is used in the CONSULTANT CODE field to show that a MIDWIFE is the responsible professional. Note that the MIDWIFE's own code is not used.

All Nursing Episodes are identified in the Admitted Patient Commissioning Data Set and Hospital Episode Statistics by a pseudo MAIN SPECIALTY CODE, 950, see Main Specialty and Treatment Function Codes Table. This includes, all Dentists, Dental Nurses, Dental Technicians, Dental Hygienists, Dental Therapists, Clinical Dental Technicians and Orthodontic Therapists

The General Dental Council Dentists Register lists all dentists registered with the General Dental Council.
The General Dental Council also maintains the General Dental Council Dental Care Professionals Register and the General Dental Council Specialist Lists in Distinctive Branches of Dentistry.

Note: GENERAL DENTAL COUNCIL REGISTRATION NUMBERS vary in length. Filling zeros are used between the prefix and GENERAL DENTAL COUNCIL REGISTRATION NUMBER, where required, to maintain total length of eight characters.

For the purposes of sending Commissioning Data Set messages to the Secondary Uses Service (regardless of how local systems hold the data), it is essential at present to continue using a 3 character field, using the first 3 characters of the ORGANISATION CODE (RESIDENCE RESPONSIBILITY) and following the same update rules relating to Prime Recipient as are currently in place. This is necessary, primarily to preserve the integrity of the current Commissioning Data Set message and the CDS PRIME RECIPIENT IDENTITY which is derived from the ORGANISATION CODE (RESIDENCE RESPONSIBILITY).
