ABOUT THE NHS DATA MODEL AND DICTIONARY VERSION 3

Change to Supporting Information: Changed Description

Following the issue of DSCN 07/2004 'Data Standards: Meta Model', the NHS Data Model and Dictionary was changed to reflect and be based upon a more generic logical data model which better supports the strategic way forward. This NHS Data Model and Dictionary was published as the NHS Data Model and Dictionary Version 3.

There was extensive quality assurance of Version 3 including involvement of the Data Definition Group. All Version 3 contents were cross mapped and checked to ensure consistency with the Version 2 contents but the underlying structure was genericised. No changes were made to NHS Business Definitions. Version 3 superseded Version 2 on 1st May 2005.

The NHS Data Dictionary Version 2 has been frozen and made available for archive information purposes only and has been watermarked accordingly. The NHS Data Dictionary Version 2 has been frozen and made available for archive information purposes only and has been watermarked accordingly.

Summary of main changes and enhancements incorporated within Version 3

  • Introduction of the capability of holding Retired Items
  • Introduction of NHS Business Definitions, which allow specific business areas to be defined in a more flexible manner
  • New ‘All Items Index’ which lists all the contents held within the NHS Data Model and Dictionary
  • Reduction in the number of data model diagrams making them more comprehensible
  • Introduction of Domains, which are conceptual logical modelling objects which identify the logical format, length and value set attributable to one or more attribute(s)
  • 'Where Used' list expanded to include a description of usage column
  • Creation of a separate Meta Model area within the publication

The introduction of Version 3 had no impact on current information standards or system suppliers.

About the Generic Model

Version 3 introduced a new Generic Model which is 'PERSON based' rather than 'ORGANISATION based'. The model has been developed around generic 'CARE ACTIVITIES' for a PATIENT, with each event transaction being recorded.

The Generic Model supports all the messages, data sets and central returns which the model supported but the underlying structure has been genericised. This involved grouping many of the old classes into generic classes like ACTIVITY GROUP. However, none of the detail was lost and most of the old class definitions can be found under 'NHS Business Definitions'. The number of diagrams has been greatly reduced and they now describe coherent areas of the model which is a far more useful approach for data modellers.

The model is aimed at enabling all 'CARE ACTIVITIES' related to the same condition for the same PATIENT to be recorded and linked across ORGANISATIONS. The different states of the same 'CARE ACTIVITY' are recorded as event transactions e.g. requested, intended, scheduled, provided, cancelled, etc. These different state events also drive the scheduling and capacity planning of resources (people, equipment, facilities, etc.) to deliver the care.

Although this may seem complicated, it is necessary both to form a coherent logical model and to relate physical information such as that which flows on the Data Sets to the logical model. Every physical item should be represented logically in the NHS Data Model and Dictionary. However, the scope of the logical model is greater than the physical information it holds and therefore not all logical information has a physical existence.

DATA COLLECTIONS HELP

Change to Supporting Information: Changed Description

A Data Set is a group of Data Elements that are required to support business analysis, for example, data associated with an Accident and Emergency Attendance is transmitted in the CDS V6-2 Type 010 - Accident and Emergency Commissioning Data Set.

The primary purpose of national data sets is to enable the same health information to be generated across the country independent of the Organisation or system that captures it.

Note: The NHS Data Model and Dictionary does not contain all national data collections.

Data Sets are grouped together on the front page of the NHS Data Model and Dictionary under Data Collections. A description of each type of data set can be found by hovering over the item name.

The Commissioning Data Sets section also contains supplementary information, see the Commissioning Data Sets Help page for further information.

Each Data Collection name is in Title Case.

Where the name of a:

  • Data Set in text is Aqua Blue, it indicates that it is a hyperlink and if clicked on will display the Data Set and
  • Central Return Form is Pink, it indicates that it is a hyperlink and if clicked on will display the Central Return Form.

There are three frames to each Data Set:

  • "Left Hand Frame": shows the appropriate Data Set menu.

  • "Middle Frame":
    • For Data Sets:
      • This displays the Data Set and may also contain information relating to the data set.
      • The Data Set is split into Data Groups which contain Data Elements grouped into logical sections.
      • If you click on any item in the Data Set, the definition will be displayed in the "Right Hand Frame".
    • For Central Return Forms, this provides a picture of the form. There is a tab for each page of the form.

  • "Right Hand Frame":
    • Initially shows an "Introduction" or "Overview" page, which provides information relating to the type of Data Set.
    • It displays Data Element definitions viewed from the "Middle Frame".

  • Where information is displayed in frames, the width of the frames can be adjusted by clicking on the “vertical” scroll bar and dragging it left or right as appropriate.

DATA ELEMENTS HELP

Change to Supporting Information: Changed Description

A Data Element is the definition of the information which is usually transmitted in a Data Set.

In addition, Data Elements can contain text providing guidance, support, values or other information concerning the Data Element and its usage.

Each Data Element:

  • Has a Green Bar to signify that it is a Data Element;
  • Name is in UPPERCASE and where the name appears in text in GREEN, it indicates that it is a hyperlink and if clicked on will display the definition for that Data Element.

The following information is available on an Data Element:

  • "Description" tab: a definition of the Data Element which provides hyperlinks to other related definitions.

    • Format/Length:
      • An entry in this field defines the format and length of the Data Element.
      • The character set used for Data Elements is the UTF-8 standard (Unicode Transformation Format-8).
      • The number indicates the field length of the Data Element.

    • National Codes:
      • An entry in this field indicates that National Codes or classifications exist for the Data Element (as an Attribute) and describes how to view them.
      • If there are no agreed National Codes or classifications for the Data Element, the field is blank.

    • Default Codes:
      • An entry in this field indicates that in addition to the agreed National Codes for the Data Element, there are default codes applicable to that Data Element only.
      • If there are no Default Codes for the Data Element, the field is blank.

    • Notes:
      • The Notes section provides guidance or other information concerning the Data Element and its usage.
      • The table at the foot of the page shows if there is an alias for this Data Element, e.g. schema name etc.

  • "Where Used" tab: every item where the Data Element is used within the NHS Data Model and Dictionary.

  • "Attribute" tab: the Attribute the Data Element is based on.
    • Most Data Elements are linked to Attributes so they can share the common Attribute definition. For example, NHS NUMBER, NHS NUMBER (BABY) and NHS NUMBER (MOTHER) are based on the one Attribute of NHS NUMBER.
    • Not all Data Elements are based on an Attribute. Some Data Elements are derived from other items, for example, AGE ON ADMISSION is a derived item and is not linked to an Attribute.

DATA MODELLING INTRODUCTION

Change to Supporting Information: Changed Description

Classes, Attributes and Relationships are components which together define the NHS Data Model and are a suggested reference point for System Suppliers when developing systems.

Note: Data Elements and Data Sets, which define the national information requirements, reference the NHS Data Model components to form part of the wider NHS Data Model and Dictionary.
  • Demonstrations

These components are explained in a short presentation. To view this presentation, see the NHS Data Model and Dictionary Components.

NHS Data Model

The NHS Data Model provides:

  • Structure to the data
    • This enables data to be seen in context to other data items. This is achieved by grouping data into Classes and having relationships between the Classes.

  • Detailed data definitions
    • Agreed definitions provide a common basis for communication.

  • High level graphical view of data
    • The NHS Data Model and Dictionary has a small set of Diagrams which represent parts of the NHS Data Model. These diagrams show the Classes, Relationships and Cardinality.

LEARNING DISABILITY

Change to Supporting Information: Changed Description

A Learning Disability (formerly known as a Mental Handicap and referred to as an Intellectual Disability) is a type of DISABILITY.

A Learning Disability usually has a significant impact on a PERSON's life. A PERSON with a Learning Disability finds it harder than others to learn, understand and communicate.

People with Profound and Multiple Learning Disabilities need full-time help with every aspect of their lives, including eating, drinking, washing, dressing and toileting etc.

Learning Disability includes the presence of a significantly reduced ability to understand new or complex information, to learn new skills (impaired intelligence), with

  • a reduced ability to cope independently (impaired social functioning)
  • which started before adulthood, with a lasting effect on development.

Further information regarding people covered by definition can be found in the document 'Valuing People - A New Strategy for Learning Disability for the 21st Century'.

Although, from a social care perspective, an Intelligence Quotient (IQ) of seventy or less is not sufficient reason for deciding if an individual should be provided with additional health and social care support, the following definition is applied within the Mental Health Services Data Set in the specific circumstance where data providers do not have an explicit data item to capture whether a PATIENT has a Learning Disability

Someone is considered to have a Learning Disability when they function at a level of intellectual ability which is significantly lower than their chronological age. This is usually considered to be equivalent to having an IQ of seventy or less:

  • Mild Learning Disability (roughly equivalent to an IQ of fifty to seventy) is comparable to the educational term 'Moderate Learning Difficulty'. It is usually caused by a combination of restricted learning and social opportunities plus a high rate of low to average intellectual ability and Learning Disability in close relatives.

  • Moderate-to-profound Learning Disability (roughly equivalent to an IQ below fifty) is comparable to the educational term 'Severe Learning Difficulty'. It usually has a specific biological cause.

Note: A Learning Difficulty is a type of Special Education Need which affects areas of learning, such as reading, writing, spelling, mathematics etc.

Further information on Learning Disabilities, can be found on the internet, for example:

PARENTING DAILY HASSLES (RETIRED)  renamed from PARENTING DAILY HASSLES

Change to Supporting Information: Changed status to Retired, Name, Description

The Parenting Daily Hassles (PDH) is a type of ASSESSMENT TOOL.This item has been retired from the NHS Data Model and Dictionary.

The aim of Parenting Daily Hassles is to assess the frequency and intensity/impact of 20 experiences that can be a 'hassle' to parents. The last live version of this item is available in the March 2018 release of the NHS Data Model and Dictionary.

The caregiver is asked to score each of the potential 'hassles' in two different ways for frequency and intensity. The frequency of each type of happening provides an 'objective' marker of how often it occurs.  The intensity or impact score indicates the caregivers 'subjective' appraisal of how much those events affect or 'hassle' them.Access to this version can be obtained by emailing information.standards@nhs.net with "NHS Data Model and Dictionary - Archive Request" in the email subject line.

The time frame for this scale can be varied according to the focus of the assessment.  For example, if a family is thought to have been under particular pressure in the last 2 months, the parent can be asked to consider how matters have been during that period.  However, if it is intended to assess progress, the same time frame should be used on each occasion.  Periods of less than one month are probably too short to give a useful picture.

Each ACTIVITY PROPERTY will have one or more ACTIVITY PROPERTY EFFECTIVE DATES and ACTIVITY PROPERTY END DATES within the CRITICAL CARE PERIOD. Both basic and advanced categories cannot be counted at the same time therefore the maximum number of systems supported is 7.
