18 WEEKS

The 18 Weeks operational standard is set out in the Operating Framework for the NHS in England published by the Department of Health and Social Care.

The 18 week referral to treatment pathway is about improving PATIENTS’ experience of the NHS - ensuring all PATIENTS receive high quality elective care without any unnecessary delay.

18 Weeks applies to pathways that do or might involve Consultant Led Activity, setting a maximum time of 18 weeks from the point of initial referral up to the start of any treatment necessary for all PATIENTS where it is clinically appropriate and where PATIENTS want it.

ABO SYSTEM

The ABO System is a CODED CLINICAL ENTRY.

The ABO System is a system of 4 basic types into which human blood may be classified according to the presence or absence of particular antigens:

  • A - Blood group A has A antigens in its red blood cells and anti-B antibodies in its plasma;
  • B - Blood group B has B antigens and anti-A antibodies in its plasma;
  • O - Blood group O blood has no antigens but both anti-A and anti-B antibodies
  • AB - Blood group AB has both A and B antigens but no antibodies, as it would destroy itself.

AUTISTIC SPECTRUM DISORDER

Autistic Spectrum Disorder is a DISABILITY.

Autistic Spectrum Disorder is defined as a lifelong condition that affects how a PERSON communicates with, and relates to, other people. Autistic Spectrum Disorder also affects how a PERSON makes sense of the world around them.

The three main areas of difficulty, which all people with Autism share, are known as the 'triad of impairments'. They are

  • difficulty with social communication (e.g. problems using and understanding verbal and non-verbal language, such as gestures, facial expressions and tone of voice)
  • difficulty with social interaction (e.g. problems in recognising and understanding other people's feelings and managing their own feelings)
  • difficulty with social imagination (e.g. problems in understanding and predicting other people's intentions and behaviour and imagining situations outside their own routine)

A CT Scan (Computerised Tomography Scan) is a Clinical Investigation.

A CT Scan uses X-rays and a computer to create detailed images of the inside of the body.

A CT Scan can be used to diagnose or monitor many different health conditions, including cancer and bone disease. It is often used to provide views of the body before another procedure takes place, such as a Biopsy or Radiotherapy treatment.

DIAGNOSTICS WAITING TIMES AND ACTIVITY DATA SET OVERVIEW

The Diagnostic waiting times reporting of the monthly waiting times and activity reporting (DM01).

The Diagnostics Waiting Times and Activity Data Set provides definitions to support the national data collections on diagnostic tests, a key element towards monitoring waits from referral to treatment. Organisations responsible for the diagnostic test activity report the diagnostic test waiting times and the number of tests completed.

The distinctions between the data set groups are not absolute and some procedures could be collected under more than one of the clinical groupings. A PATIENT waiting for a diagnostic investigation should be counted only once for each test they are waiting for, wherever the test is to be performed and even if there is any additional therapeutic intervention. Each test should be identified by their OPCS Classification of Interventions and Procedures coding where applicable.

The diagnostic investigations are grouped into categories of Imaging, Physiological Measurement and Endoscopy. The data set is for the monthly return covering 15 key diagnostic tests as below:

  • Magnetic Resonance Imaging
  • Computer Tomography
  • Non-obstetric ultrasound
  • Barium Enema
  • DEXA Scan (Dual-energy X-ray absorptiometry)

PHYSIOLOGICAL MEASUREMENT

  • Audiology - audiological assessments
  • Cardiology - echocardiography
  • Cardiology - electrophysiology
  • Neurophysiology - peripheral neurophysiology
  • Respiratory physiology - sleep studies
  • Urodynamics - pressures & flows

ENDOSCOPY

  • Colonoscopy
  • Flexible sigmoidoscopy
  • Cystoscopy
  • Gastroscopy
Aggregated numbers of PATIENTS waiting for a diagnostic test/procedure funded by the NHS should be included. This includes all referral routes (i.e. whether the PATIENT was referred by a GENERAL PRACTITIONER or by a hospital-based clinician or other route) and also all settings (i.e. Out-Patient Clinic, WARD, Imaging Department or a type of LOCATION such as a Health Centre). It is recognised that there will be some overlap between PATIENTS reported in this data set and PATIENTS reported in the other waiting times data sets.

The Diagnostic Census of the waiting times for DIAGNOSTIC TEST REQUESTS.

The Diagnostics Waiting Times Census Data Set provides definitions to support the national data collections on diagnostic tests, a key element towards monitoring waits from referral to treatment. This is a census of diagnostic test waiting times.

The diagnostic investigations are grouped into categories of Endoscopy, Imaging, Pathology and Physiological Measurement.

The distinctions between these groups are not absolute and some procedures could be collected under more than one of the clinical groupings. A PATIENT waiting for a diagnostic investigation should be counted only once for each test they are waiting for, wherever the test is to be performed and even if there is any additional therapeutic intervention. It is recognised that there will be some overlap between PATIENTS reported on this census and PATIENTS reported in the inpatient and outpatient waiting times returns.

HOSPICE

A Hospice is an Organisation.

A Hospice:

  • provides a range of SERVICES for conditions where curative treatment is no longer an option, and people are approaching the end of their life
  • provides care, treatment and support for people and their families and carers, including respite care for people who live with friends or family at home

PROVISIONAL REPLACEMENT CERTIFICATE

A Provisional Replacement Certificate (PRC) provides the same access to free healthcare as a European Health Insurance Card (EHIC).

Overseas Visitors receiving clinically necessary treatment, who fail to provide a European Health Insurance Card, have to obtain a Provisional Replacement Certificate from the appropriate liaison body in the competent member state country.

Note: if  a European Health Insurance Card (EHIC) is lost or stolen while the PERSON is abroad, they will apply for a Provisional Replacement Certificate. The Provisional Replacement Certificate provides the same cover as the European Health Insurance Card until the PERSON returns home.

QUARTERLY BED AVAILABILITY AND OCCUPANCY DATA SET OVERVIEW

The Quarterly Bed Availability and Occupancy Data Set (KH03) requires summary information from NHS Health Care Providers for Hospital Bed availability and occupancy. It identifies the number of available and occupied bed days for each NHS Health Care Provider

RECIPROCAL HEALTHCARE AGREEMENT

  • If the Rh factor is present, the PERSON's blood group is RhD positive;
  • If the Rh factor is absent, the PERSON is RhD negative.

This means that a PERSON can be one of eight blood groups:

  • A RhD positive (A+)
  • A RhD negative (A-)
  • B RhD positive (B+)
  • B RhD negative (B-)
  • O RhD positive (O+)
  • O RhD negative (O-)
  • AB RhD positive (AB+)
  • AB RhD negative (AB-).

Introduction:

The Systemic Anti-Cancer Therapy Data Set collects clinical management information on PATIENTS undergoing Chemotherapy in (or funded by) the NHS in England.

In the clinical setting its primary use will be in prescribing and administering Chemotherapy. The standard also specifies secondary uses information about systemic anti-cancer therapy (Chemotherapy) which is required to assist in achieving, supporting and monitoring the NHS Operating Framework, specialist commissioning and related policy.

Note: the term “Chemotherapy” is used synonymously with Systemic Anti-Cancer Therapy as it is the term in common usage.

Details of Standard:

The Systemic Anti-Cancer Therapy Data Set relates to all cancer PATIENTS, both adult and paediatric, in both admitted PATIENT care and outpatient and community settings, who are receiving Chemotherapy treatment for all solid tumour and haematological malignancies, including those treated in clinical trials.

The Systemic Anti-Cancer Therapy Data Set covers the period from the Start Date of the Systemic Anti-Cancer Drug Programme to the Start Date of the last Systemic Anti-Cancer Drug Cycle.

Note: PATIENTS may end their treatment with an oral or other component taken at home. Although the dispensing of this will be recorded in the final Systemic Anti-Cancer Drug Cycle, it is not possible to confirm that the PATIENT has taken the medication.

Data Set Structure and Transmission:

Chemotherapy is given over a prolonged period of time, often months or years, comprising repeating and sequential elements.  The PATIENT may attend two or more Health Care Providers during the course of treatment.

In order to track the PATIENT during treatment, the data set must be capable of linking all the elements of care in a consistent and ordered way. In order to achieve this, the Systemic Anti-Cancer Therapy Data Set has a branching structure which links the initial data fields, which will remain constant during the treatment, with detail of each Systemic Anti-Cancer Drug Regimen, Systemic Anti-Cancer Drug Cycle and drug administration.  At the completion or cessation of a Systemic Anti-Cancer Drug Regimen, the outcome section must link back to all previous fields. It must be possible to reconstitute details of each PATIENT’s sequential management from the serial downloads received.

All NHS Trusts and NHS Foundation Trusts providing cancer Chemotherapy SERVICES are required to submit monthly data downloads to an agreed timetable, one month in arrears. These data must represent all treatment activity in the month period, including Systemic Anti-Cancer Drug Regimens started and completed or ceased in that time period.

The data repository is hosted by the National Cancer Registration and Analysis Service (NCRAS) Oxford, (formerly the Oxford Cancer Intelligence Unit), and the data is held under their section 251 of the National Health Service Act 2006.

Data downloads will be either the csv or XML format and will uniquely identify the provider.

The COSDS UNIQUE IDENTIFIER is used in conjunction with the ORGANISATION CODE (CODE OF PROVIDER) to uniquely identify a record within a Cancer Outcomes and Services Data Set submission to the National Cancer Registration and Analysis Service.The COSDS UNIQUE IDENTIFIER is used in conjunction with the ORGANISATION IDENTIFIER (CODE OF PROVIDER) to uniquely identify a record within a Cancer Outcomes and Services Data Set submission to the National Cancer Registration and Analysis Service.

The COSDS UNIQUE IDENTIFIER may appear on data quality reports, error reports, and audit logs exchanged between the National Cancer Registration and Analysis Service and the submitting ORGANISATION CODE (CODE OF PROVIDER).The COSDS UNIQUE IDENTIFIER may appear on data quality reports, error reports, and audit logs exchanged between the National Cancer Registration and Analysis Service and the submitting ORGANISATION IDENTIFIER (CODE OF PROVIDER).

The COSDS UNIQUE IDENTIFIER must be populated for each record, in the COSDS Record Identifier group within the COSDS Message, by the sender of the data set submission prior to transmission of the data to the National Cancer Registration and Analysis Service.

EARLIEST REASONABLE OFFER DATE is the date of the earliest of the Reasonable Offers made to a PATIENT for an APPOINTMENT or Elective Admission.EARLIEST REASONABLE OFFER DATE is the DATE of the earliest of the Reasonable Offers made to a PATIENT for an APPOINTMENT or Elective Admission. It should only be included on the Commissioning Data Sets where the PATIENT has declined at least two Reasonable Offers, and a Patient Pause is to be applied to the length of wait calculation performed by the Secondary Uses Service.

For an APPOINTMENT this is the earliest of the APPOINTMENT DATES OFFERED where the REASONABLE OFFER INDICATOR of the APPOINTMENT OFFER is National Code 'Reasonable Offer'.

For an OFFER OF ADMISSION this is the earliest of the OFFERED FOR ADMISSION DATES where the REASONABLE OFFER INDICATOR of the OFFER OF ADMISSION is National Code 'Reasonable Offer'.

Patient Cancellations
Where, for any reason, a PATIENT cancels or does not attend an APPOINTMENT or an OFFER OF ADMISSION the EARLIEST REASONABLE OFFER DATE for the rearranged APPOINTMENT or OFFER OF ADMISSION will be the EARLIEST REASONABLE OFFER DATE of the cancelled APPOINTMENT or OFFER OF ADMISSION.

Provider Cancellations
Where, for any reason, any Health Care Provider cancels and re-arranges an APPOINTMENT or an OFFER OF ADMISSION, the EARLIEST REASONABLE OFFER DATE for the re-arranged APPOINTMENT or OFFER OF ADMISSION will be the date of the earliest Reasonable Offer made following the cancellation.

Patients who are unavailable
Where a PATIENT makes themself unavailable for a longer period of time, for example a PATIENT who is a teacher who wishes to delay their admission until the summer holidays, making a Reasonable Offer may be inappropriate.

In these circumstances, so long as the Health Care Provider could have made at least two Reasonable Offers, the EARLIEST REASONABLE OFFER DATE will be the date of the earliest Reasonable Offer that the provider could have offered the PATIENT. This must be communicated to the PATIENT.

Use in Commissioning Data Set version 6-0 onwards for Referral To Treatment Consultant-Led Waiting Times:

If the Commissioning Data Set record:

relates to a Referral To Treatment Period Included In Referral To Treatment Consultant-Led Waiting Times Measurement

and

includes the REFERRAL TO TREATMENT PERIOD END DATE of the REFERRAL TO TREATMENT PERIOD

and

is of the following Commissioning Data Set Types:

then EARLIEST REASONABLE OFFER DATE must be populated in the Commissioning Data Set record if a Patient Pause (the PATIENT is paused on the ELECTIVE ADMISSION LIST because they have made themselves unavailable for treatment for a specified period (for non-clinical reasons)) is to be applied to a REFERRAL TO TREATMENT PERIOD by the Secondary Uses Service

Failure to include EARLIEST REASONABLE OFFER DATE in the Admitted Patient Care General Episode Commissioning Data Set record carrying the REFERRAL TO TREATMENT PERIOD END DATE, will mean no Patient Pause is applied to the duration of wait calculation for the REFERRAL TO TREATMENT PERIOD performed by the Secondary Uses Service.

Use in the Community Services Data Set, Mental Health Services Data SetCommissioning Data Sets (version 6-2 onwards) for Allied Health Professional Referral To Treatment:Use in the Community Services Data Set, Mental Health Services Data Set, Commissioning Data Sets (version 6-2 onwards) for Allied Health Professional Referral To Treatment:

For the Community Services Data Set, Mental Health Services Data Set and the Commissioning Data Sets (version 6-2 onwards) the EARLIEST REASONABLE OFFER DATE may be used locally to inform waiting time calculations for Allied Health Professional Referral To Treatment Measurement.  It can be used to account for periods of time where the PATIENT has not accepted the first available APPOINTMENT OFFER and this has extended the Allied Health Professional Referral To Treatment Measurement waiting time, for example:

  • where a PATIENT who is a child has been offered an APPOINTMENT but their parent/carer states that they wish to wait until the school holidays commence. The SERVICE cannot commence planned treatment until the PATIENT is available.
  • where the PATIENT works away and cannot attend for a period of time, but it is not appropriate to discharge the PATIENT from the SERVICE.
