The 18 week referral to treatment pathway is about improving PATIENTS’ experience of the NHS - ensuring all PATIENTS receive high quality elective care without any unnecessary delay.

18 Weeks applies to pathways that do or might involve Consultant Led Activity, setting a maximum time of 18 weeks from the point of initial referral up to the start of any treatment necessary for all PATIENTS where it is clinically appropriate and where PATIENTS want it.

Every secondary care provider in England has to send a set of standard Commissioning Data Sets to the Secondary Uses Service which contains details of all the care they have provided.

Contextual Overview

The NHS need to measure and monitor the REFERRAL TO TREATMENT PERIOD within PATIENT PATHWAYS to ensure that they are progressing as planned to achieve the 18 weeks target.

In an estimated 10% to 20% of cases, responsibility for the PATIENT PATHWAY will be transferred between Health Care Providers. The receiving Health Care Provider would be unable to report on the 18 weeks target for these cases unless the referring Health Care Provider supplied the PATIENT PATHWAY information at the time of transfer.

Lower Layer Super Output Areas are a geographic hierarchy designed to improve the reporting of small area statistics in England and Wales.

Lower Layer Super Output Areas are built from groups of contiguous Output Areas and have been automatically generated to be as consistent in population size as possible, and typically contain from four to six Output Areas. The Minimum population is 1000 and the mean is 1500.

There is a Lower Layer Super Output Area for each POSTCODE in England and Wales. A pseudo code is available for Scotland, Northern Ireland, Channel Islands and the Isle of Man.

MIXED-SEX ACCOMMODATION DATA SET OVERVIEW

The Mixed-Sex Accommodation Data Set collects performance information on a monthly basis from Health Care Providers of NHS funded care, including independent sector and social enterprise/voluntary organisations, on the number of occurrences of breaches of the sleeping accommodation guidance. However the count of occurrences of breaches exclude private and self-funded PATIENTS in NHS Health Care Providers.

A breach occurs at the point a PATIENT is admitted to mixed-sex accommodation or moves to mixed-sex accommodation from Same Sex Accommodation.

Breaches of bathroom accommodation, including situations where a PATIENT must pass through opposite gender areas to reach their own facilities, and no provision of women-only lounges in mental health units, must be recorded at organisation level, and plans put in place to deal with the problem. However these types of breaches are not reported in the Mixed-Sex Accommodation Data Set.

National Cancer Waiting Times Monitoring Data Set

Concept of Operation and Patient Pathway Scenarios:

The National Cancer Waiting Times Monitoring Data Set is a generic data set designed to support the monitoring of waiting times for a variety of different pathways of cancer care. For the purpose of this data collection cancer is defined using the International Classification of Diseases (ICD) codes. The full list of International Classification of Diseases (ICD) diagnosis codes is available on the NHS Digital website at: Cancer Waiting Times.

Collection and submission of the National Cancer Waiting Times Monitoring Data Set is to be managed according to the maximum waiting time and information requirements of the pathway of care for each individual PATIENT. These requirements for providers of cancer SERVICES to return data to the Cancer Waiting Times Database are defined using different scenarios.

The columns in the table below show which data items are required for a range of health care scenarios:

Data Set Notation:

  • M = Mandatory: the Standard Contract Schedule 5 requires NHS provider Organisations to submit this information on a monthly basis. NHS England require the data to be submitted 25 working days after the end of each month or quarter.

Introduction

The Neonatal Critical Care Minimum Data Set (NCCMDS) provides a record of what happens to a PATIENT when they receive Neonatal Critical Care in a Neonatal Intensive Care Unit, Maternity WARD, or Neonatal Transitional Care WARD.

The primary purpose of the Neonatal Critical Care Minimum Data Set is to allow the operation of the National Tariff Payment System within neonatal critical care. The Neonatal Critical Care Minimum Data Set supports the National Tariff Payment System by capturing the data needed to generate a Healthcare Resource Group (HRG) for each calendar day (or part thereof) of a period of neonatal critical care; these Healthcare Resource Groups are then used to inform the annual aggregate costing exercise, NHS Reference Costs.

The scope of the Neonatal Critical Care Minimum Data Set is:

a) All PATIENTS on a WARD with a CRITICAL CARE UNIT FUNCTION of National Code '13 - Neonatal Intensive Care Unit (Neonatal critical care patients predominate)'.

b) All PATIENTS (excluding Mothers) on a WARD with a CRITICAL CARE UNIT FUNCTION of National Code:

  • 14 - Facility for Babies on a Neonatal Transitional Care Ward
  • 15 - Facility for Babies on a Maternity Ward

to whom one or more of the following CRITICAL CARE ACTIVITY CODES applies for a period greater than 4 hours:

01Respiratory support via a tracheal tube (Respiratory support via a tracheal tube provided)
02Nasal Continuous Positive Airway Pressure (nCPAP) (PATIENT receiving nCPAP for any part of the day)
04Exchange Transfusion (PATIENT received exchange transfusion)
05Peritoneal Dialysis (PATIENT received Peritoneal Dialysis)
06Continuous infusion of inotrope, pulmonary vasodilator or prostaglandin (PATIENT received a continuous infusion of an inotrope, vasodilator (includes pulmonary vasodilators) or prostaglandin)

Ultrasound Scans can be used:

  • to study blood flow and to detect any narrowing or blocking of blood vessels;
  • during a Maternity Episode to produce images of the baby in the womb.

COMMUNITY HEALTH INDEX NUMBER

The COMMUNITY HEALTH INDEX NUMBER (CHI NUMBER) uniquely identifies a PATIENT on the Community Health Index (Scotland) within the NHS in Scotland.  It is the equivalent of the NHS NUMBER in England and Wales.

The COMMUNITY HEALTH INDEX NUMBER is ten numeric digits in length, and is constructed as follows:

  • PERSON BIRTH DATE (in format DDMMYY);
  • two further digits;
  • a ninth digit which is always an even number for females and an odd number for males;
  • an arithmetical check digit.

ORGANISATION CODE (CODE OF COMMISSIONER) is the ORGANISATION CODE of the Organisation commissioning health care.

For Commissioning Data Sets, the ORGANISATION CODE (CODE OF COMMISSIONER) should always be the ORGANISATION CODE of the original commissioner to support the National Tariff Payment System.

The NHS England document "Who pays?The NHS England document "Who pays? Determining responsibility for payments to providers" sets out a framework for establishing responsibility for commissioning an individual's care within the NHS, (i.e. determining who pays for a PATIENT’s care.)

Where NHS England is the responsible commissioner for a specialised SERVICE, based on the NHS England Commissioner Assignment Method (CAM), one of the Specialised Commissioning Hub ORGANISATION CODES should be used depending on which Health Care Provider delivered the SERVICE, e.g. NHS Trust, Independent Sector Healthcare Provider.

The NHS Digital website provides a mapping list of which Health Care Providers map to which Specialised Commissioning Hub. The mapping can be found on the Organisation Data Service web pages at: Provider to Commissioning Hub Mapping.

ORGANISATION CODE (CODE OF COMMISSIONER) will be replaced with ORGANISATION IDENTIFIER (CODE OF COMMISSIONER), when it has been approved for use in national information standards.
